The Delta Initiate delivered preventative and clinical health services for individuals with, or at risk of developing, chronic diseases in rural Tennessee.
To aid the program in effective service delivery and evaluation, Data Works, LLC designed and maintained data collection tools to aid program personnel in tracking services and client progress; provided technical assistance and support to program personnel to ensure accurate data collection; and assessed the quality of services and effectiveness of program activities through survey design and administration and data collection and analysis.
Data Works, LLC conducted both process and outcome evaluations to promote program quality, measure efficacy, and meet program standards, goals, and objectives; and prepared and submitted quarterly and annual reports to inform key personnel of the program’s effectiveness in improving clients’ access to quality healthcare and clients’ overall health.
